The real question is whether you're in the small group that's short. Four groups run low: vegetarians and vegans, because plant zinc is locked in phytate that blocks a third to a half of it 1; adults over 65, where 30 to 45% run mildly deficient 2; anyone on long-term acid blockers like omeprazole, which strip the acid zinc needs; and people with bowel disease, celiac, or bariatric surgery. Everyone else eating meat weekly is covered.
The strongest evidence is about colds, and it isn't the daily pill. Zinc lozenges started within 24 hours of a sore throat cut a cold from seven days to four 34. The zinc has to dissolve in your mouth and coat the throat where the virus multiplies; a swallowed daily pill lands in the wrong place, so the winter prevention habit is the bit the evidence won't back.
The testosterone claim is real only if you're deficient. Severe restriction tanks testosterone, and repletion in low men brings it back 5. In men who start normal, extra zinc does nothing, and chasing it at 50 to 100 mg buys the copper problem below for no gain.
Inflammatory acne responds too: 30 mg a day roughly halves red-bump counts over two to three months 6, close behind the antibiotic dermatologists try first 7.
If you're in a low group, three decisions: form, dose, timing.
What repletion does, if you were low:
- Weeks 2 to 4. Taste comes back, food gets interesting, appetite ticks up 10.
- Weeks 4 to 8. Wounds close on schedule; inflammatory acne is visibly better 7.
- Months and years. Fewer, milder colds for the marginal older adult 11; drifting systems settle.
If you weren't low, none of it happens; there was nothing to fix.
The fine print — when to skip it, and what people get wrong
"Oxide is fine, it's mostly zinc." The label milligrams don't absorb; oxide needs strong stomach acid and gives near-nothing on acid blockers. "Picolinate is better absorbed" rests on one 1987 study of fourteen people, never replicated 12.
Never put zinc in your nose — intranasal Zicam was pulled after 130-plus reports of permanent smell loss 13. Don't run above 40 mg/day for months without added copper 9. In pregnancy, stay within the prenatal's dose.
Chronic high-dose zinc blocks copper for months, and copper loss damages spinal nerves — cases of numb feet after years on 100-plus mg 14. Recovery is often incomplete.

Zinc
Generic zinc gluconate or citrate at 15–25 mg/day runs $10–25/year — cheapest tier of supplementation.
One tablet daily, ideally with food and separated from iron/calcium and tetracyclines; minor lifestyle adjustment.
Strong RCT evidence for the lozenge cold-duration effect (Cochrane, individual-patient meta-analysis) and for the AREDS/AREDS2 AMD indication; deficiency biology is settled (IOM 2001). Daily oral supplementation in replete adults has weaker, more contested evidence, and the testosterone narrative is widely overstated. Form differences (Wegmüller et al. 2014) are now well-characterised; picolinate's superiority claim rests on a single 1987 study.
Oral zinc gluconate ~30 mg/day reduces inflammatory acne lesion counts ~30–50% over 8–12 weeks across ~25 reviewed trials (Cervantes 2018; Andrews 2014); head-to-head vs. minocycline showed 49% vs. 63% response in moderate acne (Dréno et al. 2001) — real visible skin effect within weeks, modest in magnitude.
Repletion in marginal deficiency restores taste, appetite, and wound-healing within weeks (Henkin 1976; Hambidge & Krebs 2007); lozenge protocol cuts cold duration ~33–40% (Singh & Das 2013; Hemilä et al. 2017) but this is acute-treatment, not daily-supplement, evidence — daily oral zinc in replete adults shows no consistent symptom benefit.
Correction of marginal deficiency supports hair, nail, and wound-healing integrity through keratinocyte differentiation and matrix metalloproteinase activity (Cervantes 2018), but most replete Western adults will see no long-term aesthetic trajectory difference.
AREDS-formula zinc reduces 5-year progression to advanced AMD ~25% in intermediate AMD (AREDS 2001; AREDS2 2013) — meaningful but narrow indication. Childhood diarrhoea mortality reduction (Lazzerini & Wanzira 2016) is large but population-specific. For the typical Western adult, longevity effect is marginal.
Deficiency correction lifts fatigue and exercise tolerance in marginally-zinc-deficient older adults (Hambidge & Krebs 2007); zinc is not a stimulant and provides no energy benefit in replete adults.
Small adjunct effect with SSRIs in major depression in limited trials; mechanism plausible via NMDA modulation and BDNF (Prasad 2008), but RCT base thin and effect small in non-deficient populations.
