The diabetes signal is the strong one. A daily serving of plain yogurt tracks with about 18% lower odds of developing type 2 diabetes; pool fourteen cohorts across the US, Europe, and Asia and the risk lands near 0.82 1. Milk and cheese don't carry it the same way, which is why the FDA now lets the label say regular yogurt may lower type 2 diabetes risk 2.
It plateaus early. Half a cup a day captures most of the effect; bigger servings don't stack more benefit 3. A cup a day also tracks with ~11% lower all-cause mortality 4. And across 120,877 adults followed twenty years, yogurt was the single food most linked to less long-term weight gain, ahead of vegetables and nuts 5.
The bacteria do your digesting. The starter cultures carry their own lactose-splitting enzyme and release it in your small intestine, finishing the milk sugar your gut can't 6. Most adults worldwide lose that enzyme after childhood, so a cup of plain yogurt goes down clean where a glass of milk sets them off. A cup is also ~200 mg calcium and 6–10 g protein, double when strained.
The rule is on the ingredient list, not the front of the tub.
What you get, and when.
- First week: if milk used to leave you bloated or cramping, that stops. Breakfast can be cold and creamy again.
- A month or two: swap it for the afternoon biscuits and the four-o'clock crash quiets down, mostly the protein.
- A decade: about a fifth fewer of the people doing this get type 2 diabetes 1, and their year-to-year odds of dying sit roughly a tenth lower 4.
The cheap cup in the fridge is doing some of that work, every day, for decades.
The fine print — when to skip it, and what people get wrong
A fruit cup packs 15–25 g added sugar, a soda's worth, and inherits none of the benefit. "Probiotic" is mostly a sticker 7. The villain is sugar; whole-milk yogurt is fine 1.
The cultures don't move in and stay; they clear within days of stopping 8. The benefit is in the doing — treat it like flossing, not a one-time reset.

Plain Yogurt
Plain yogurt is $0.50-$2/serving; a daily small cup runs ~$200-$400/year. Trivially low against the catalogue's ladder.
Trivial — a daily habit of buying plain unsweetened yogurt and eating ~100-170 g. The only real effort is resisting the sweetened-yogurt aisle.
Three large US cohorts pooled (Chen et al. 2014), a 17-cohort dose-response meta-analysis (Aune et al. 2013), and a mortality meta-analysis (Salari-Moghaddam et al. 2023) all align; the FDA QHC formalises 'limited but credible' evidence (FDA 2024). Plus the settled biochemistry of yogurt-mediated lactose digestion (Savaiano 2014). No long-term RCT with hard endpoints — that's the ceiling on the score.
Felt within weeks for lactose maldigesters (the world's ~65-70% non-lactase-persistent majority) — yogurt's microbial beta-galactosidase digests residual lactose in the duodenum (Savaiano 2014). Gut comfort improves modestly in adults consuming fermented milk with B. lactis CNCM I-2494 (Eales et al. 2017). Greek/strained styles add satiety via protein density. Small but real day-to-day improvements.
Pooled cohort data give HR 0.89 for all-cause and cardiovascular mortality at 200 g/day, with the dose-response curve plateauing past ~0.5 serving/day (Salari-Moghaddam et al. 2023). One serving/day is associated with RR ~0.82 for incident T2D across 14 cohorts (Chen et al. 2014); FDA issued a Qualified Health Claim in March 2024 (FDA 2024). Observational, residual confounding likely, no incident-T2D RCT — meaningful but modest.
Modest contribution to long-term appearance via bone density preservation (Sahni et al. 2013), satiety-mediated weight regulation (Mozaffarian et al. 2011), and gut comfort. Not a beauty driver; the visible effect is the slow background of being a steadier metabolic baseline over decades.
