Mitochondria are the cell's little engines, and how many you have isn't fixed. A master gene switch, PGC-1α, tells the cell to build more when it gets flipped on. Exercise is the most reliable way to flip it. In cell culture, PQQ flips the same switch 1. It's also a redox cycler that mops up cell-damaging free radicals thousands of times over without falling apart 2. The cell biology is solid. Whether you feel any of it is the open question.
Humans are harder than dishes. Five small trials carry the published evidence, and they point the same way but quietly. In tired adults, 20 mg a day for eight weeks nudged fatigue, mood, and sleep-onset subscales 3. In adults fifty to seventy, 20 mg a day for twelve weeks improved attention and working memory 4. Three days of dosing lowered inflammation markers 5. The trials run 10 to 41 people, most were paid for by the patent holder, and the one independent trial found nothing on its main endpoint 6. Real, but modest, and not enough to bet anything important on.
If you try it, run it as a single clean experiment. Every trial that found something used 10 to 20 mg a day; more doesn't do more.
What you'd actually notice, anchored to what the trials measured.
- First days: nothing. There's no acute hit; a week-one buzz is placebo 5.
- Six to eight weeks: if anything comes, it's here. For someone walking in tired, a small lift: afternoons drag less, the bedside lamp goes off ten minutes sooner 3. Your partner won't notice; you might.
- Three months, past fifty: a modest bump in attention and recall, the names coming back a little faster 4.
- Years: no long-term trials exist. The mechanism is consistent with continued support; the outcomes are unknown.
The fine print — when to skip it, and what people get wrong
What the bottle gets wrong. It isn't a B vitamin: that claim was proposed and rebutted the same year 7 8. "Thousands of times stronger than vitamin C" is true per molecule, meaningless at milligram doses. And it doesn't replace CoQ10, which works inside existing mitochondria.
Skip it in pregnancy and breastfeeding: no supplement-dose trials exist in either. No documented drug interactions, but none have been formally studied, so clear it with your pharmacist if you're on prescription medication.
Two ways it disappoints: pushing to 40–60 mg for a lift that isn't coming, which only adds stomach upset; and expecting it to help a healthy young person whose mitochondrial reserve is already full, where the negative trial ran 6.

Harris et al. 2013 showed reduced CRP and IL-6 within 72 hours at 0.2 mg/kg, plus urinary metabolite shifts consistent with mitochondrial-related metabolism (Harris 2013); Nakano 2012 showed open-label improvements in fatigue and tension/anxiety on POMS over 8 weeks (Nakano 2012). Small, modest, real-but-quiet.
Nakano 2012 (open-label, N=17, 8 weeks) showed improved vigour and reduced fatigue on POMS subscales in adults with self-reported baseline fatigue. Effect is modest and largely subjective; Hwang 2020 saw no aerobic-performance benefit in young untrained men.
Itoh et al. 2016 (RCT, N=41, 12 weeks) found improved attention and working-memory subscale scores in adults 50–70 on 20 mg/day BioPQQ, with corroborating fNIRS prefrontal-activation shifts. Modest, replicated in Koikeda 2011's Stroop result; effect concentrated in older adults.
Nakano 2012 showed improved sleep onset and sleep duration on OSA-MA questionnaire over 8 weeks at 20 mg/day in adults with self-reported poor sleep. Open-label, subjective endpoint, no polysomnography; effect is real-but-modest in the deficit population.
Nakano 2012 showed improved tension/anxiety POMS subscale alongside vigour and fatigue improvements. Open-label and small; not on the level of an anxiolytic but a measurable subjective shift in a fatigued population.
Five human trials (N=10–41 each), four industry-funded, mechanism strong (Chowanadisai 2010, Stites 2006 replicated) but human outcome data sparse. The one independent RCT (Hwang 2020) was negative on its primary endpoint. Plausible mechanism, thin human translation.
Mechanism (PGC-1α-driven mitochondrial biogenesis, Chowanadisai 2010) is the same pathway exercise activates and is mechanistically tied to age-related mitochondrial decline, but no human longevity or hard-endpoint outcome data exist; mitochondrial density in humans on PQQ has never been directly measured.
