The salt swap is the strongest single move. Rural-Chinese adults at high stroke risk who cooked with a 75% salt, 25% potassium-chloride blend instead of ordinary salt had 14% fewer strokes and 12% lower overall mortality over five years 1. Other populations show the same direction 2. It's one product on one shelf.
Blood pressure is the lever underneath it. Raising potassium to guideline levels drops systolic pressure about 3 points in healthy adults and closer to 7 in people already running high 3. Seven points is what a starter blood-pressure pill buys, except the source is your plate.
It's also the best-studied thing for kidney stones. In two large Harvard cohorts, the people eating the most potassium had 35% to 51% fewer first stones 4 5. Potassium pulls citrate into the urine, where it grabs calcium before calcium can find oxalate and start a stone.
Add one potassium food to most plates, then fix the shaker. Food closes most of the gap; the salt swap covers the rest.
Do the food first, the shaker second. Together they clear the 3,400 mg target without anyone counting milligrams 6.
The cuff moves first, then the odds.
- Two to four weeks: systolic pressure starts drifting down, 5 to 10 points if you were high, 2 or 3 if you weren't 3. You won't feel it; a home monitor shows it.
- Several years: the stroke and heart-event curves bend, and the gap widens the longer you keep it up 1.
- Decades: fewer strokes, fewer stones. It arrives as the absence of bad news rather than any feeling of change.
The fine print — when to skip it, and what people get wrong
Healthy kidneys dump extra potassium easily; whole food is essentially risk-free. The concentrated salt substitute is the part to check on.
The banana is a middling source (420 mg); potatoes, beans, spinach, and avocado all carry more. Muscle cramps are mostly not a potassium problem; sweat is sodium-heavy, and cramps trace to tired misfiring muscle, not diet 7. Pills aren't food potassium; capped at 99 mg a dose, they skip the citrate and fibre that come packaged with a potato.

Potassium
Beans, potatoes, leafy greens are among the cheapest foods per gram of potassium delivered. A canister of 75/25 NaCl/KCl salt substitute runs $3-6 and lasts months.
SSaSS (n=20,995, 4.7 yr) showed 12% lower all-cause mortality, 14% fewer strokes, 13% fewer major cardiovascular events with salt substitution (Neal 2021). Meta-analytic BP reductions of ~6.8 mmHg systolic in hypertensives at 3,500 mg/day intake (Filippini 2020) translate into population-level cardiovascular benefit. Not a 5 because the definitive mortality RCT was conducted in a high-risk Chinese rural cohort; generalisability to a normotensive Western reader rests on mechanism + dose-response rather than direct trial.
Either a mild dietary shift (one whole-food side per meal) or a one-time swap of the salt shaker. Sustained but not demanding; SSaSS adherence was high across five years (Neal 2021).
Multiple high-quality meta-analyses of BP-lowering RCTs (Whelton 1997, Aburto 2013, Filippini 2020) plus a definitive cardiovascular-mortality RCT (SSaSS, Neal 2021) plus DASH/DASH-Sodium trials. Direction is consensus and guideline-backed (WHO, AHA, NASEM directionally). NASEM 2019 softened the chronic-disease-prevention threshold because precise dose-response in well-nourished populations is less settled — keeps the rating off 5.
Within 2-4 weeks of adequate intake, systolic BP drifts down by ~2-3 mmHg in normotensives and 5-10 mmHg in hypertensives (Filippini 2020). Real but silent for almost all readers — a measurement change rather than a felt one.
