Your gut can't tell phytosterols from cholesterol. Cholesterol has to hitch a ride in tiny fat-carriers in your intestine before it crosses into your blood, and there are only so many seats. Phytosterols take the seats; the displaced cholesterol leaves in stool, so your liver pulls more LDL from the blood to cover the shortfall 1. The phytosterols themselves barely get in; a dedicated pump shoves them straight back out 2.
The LDL drop is one of the most-replicated findings in nutrition. Over a hundred trials land on the same curve, flattening by 3 grams a day 3. It even stacks on a statin, adding roughly another 9 percent for someone who can't quite hit target 4.
But every endorsement rests on the number, not on hearts. The FDA, EFSA, and the cholesterol guidelines cleared phytosterols on the LDL drop alone; no one has counted heart attacks 5. The predicted benefit is borrowed from statin math, where each big LDL cut buys about a fifth fewer vascular events 6. Here, that prediction is all you're getting, and the gap behind it is unusually wide.
The dose is 2 grams a day, with a meal that has some fat in it. Higher buys nothing 7.
The invisibility is the real adherence problem. The people who stick with it are the ones getting a lipid panel every six to twelve months to turn the habit into a number on a page.
Know where this sits before you commit. Phytosterols are the modest rung. A statin cuts LDL 30 to 55 percent with decades of trials proving fewer heart attacks and deaths 6. Ezetimibe, the prescription cousin blocking the same gut step, cuts about 18 percent with proven event reduction. Soluble fiber (psyllium, oat) adds another 5 percent and layers cleanly. If your risk warrants a statin and you're using phytosterols to dodge one, the math isn't in your favour 8.
The fine print — when to skip it, and what people get wrong
You can't hit the dose from food; nuts and seeds top out near 400 mg against the 2,000 you need 1. Stanols and sterols work the same at matched dose 3. Genetic studies hint lifelong high plasma sterol tracks with heart disease, though there's no evidence of harm at label doses 9.
Sitosterolemia, the rare inherited condition where the plant-sterol pump is broken, is the hard no 2. Skip supplements in pregnancy and breastfeeding, where there's no safety data 1. Everyone pays one small cost: carotenoid absorption drops about 15 percent, fixed by a carotenoid-rich vegetable the same day 10.

Phytosterols
Phytosterol capsules ~$50-100/year; enriched margarines and yogurt drinks at the upper end of $150-300/year. Falls in the $50-500/year minor band.
2 g/day with a fat-containing meal, indefinitely. A daily fortified product or capsule, no preparation, but adherence is invisible (no felt feedback) which makes the habit fragile without an external feedback loop (semi-annual lipid panel).
Biomarker evidence is among the strongest in nutrition: Ras 2014 meta-analysis pooled 124 RCTs (>9,600 participants) showing a continuous dose-response, plateauing near 3 g/day at ~12% LDL reduction. Effect replicates additively on top of statin therapy (Han 2016). FDA-authorized health claim since 2000; EAS Consensus 2014 endorsement (Gylling 2014). Capped at 4 because no event-powered RCT exists; the LDL-to-outcome step is extrapolated, not measured.
Lowers LDL cholesterol by 8-10% at 2 g/day across >100 RCTs (Ras 2014, Demonty 2009); LDL is the most validated causal mediator of atherosclerotic cardiovascular disease (CTT 2010). No hard-endpoint trial has been conducted with phytosterols, and Mendelian-randomization analyses show genetically elevated serum sitosterol predicts higher coronary risk (Helgadottir 2022, Yang 2023), discounting the surrogate-to-outcome extrapolation. Net effect: a real but modest contribution to long-term cardiovascular risk reduction, primarily as adjunct to other LDL strategies.
