Three things happen in the cup, on different clocks. L-theanine crosses into the brain and lifts the alpha rhythm of being relaxed and alert at once, while the caffeine wakes you the usual way 1. The catechins, green tea's polyphenols led by EGCG, push the liver to pull more cholesterol from the blood. And before you swallow, those same catechins bind and kill the bacteria that build plaque and drive gum disease 2.
The randomized signal is small but it replicates. Daily tea lowers LDL by about 2 mg/dL and total cholesterol by around 7, sparing HDL 3, and drops blood pressure roughly two millimetres, most in people who started high 4. Neither moves a clinical decision alone; both run the right way, cheaply.
The mortality signal is observational but large. Japanese adults drinking five or more cups a day had all-cause death rates 15–25% lower than near-abstainers, strongest on stroke, replicated across eight pooled cohorts of over three hundred thousand people 5 6. The acute focus effect is the cleanest: two cups' worth of caffeine plus theanine sharpened accuracy on a demanding attention-switching task an hour later, where caffeine by itself failed 7.
The dose that tracks the mortality signal is five cups a day; two or three still buys most of the cognitive and oral benefit. How you brew it matters more than people expect.
Inside an hour: alertness that is present and focused, without the wired edge; task-switching feels cleaner 7. Inside a week: the morning film on your teeth is thinner, though the dentist notices first. Inside two months: LDL and blood pressure a couple of points lower on the next reading. Across a decade: 15–25% lower mortality at five cups a day 6. You feel the first beat; you can only see the last on a population chart.
The fine print — when to skip it, and what people get wrong
Extract capsules are a different beast than the tea. Concentrated EGCG at 800 mg a day or more has caused acute hepatitis 8; brewed-tea drinkers are absent from the liver-injury registry 9. Cap at three cups in pregnancy, and flag warfarin or nadolol to your clinician.
"It burns fat." The weight-loss meta-analysis lands near 1.3 kg over months 10 11: real, not a lever. "It cures cancer." The cohort signal is positive for a few cancers and null for most; the trials never arrived 12. "Decaf is pointless." Most catechins survive decaffeination; you lose only the caffeine-theanine shift.

Green Tea
Loose-leaf or bagged green tea at typical Western retail runs $0.05–0.40 per cup; a 4-cup-a-day habit is well under $200/year. Trivial cost.
Boil water, steep 2–3 minutes, drink. The only ongoing micro-decision is the caffeine cutoff (last cup before ~14:00) and meal timing for the iron-absorption interaction.
Multiple meta-analyses of RCTs converge on small-but-significant LDL and BP effects (Zheng 2011; Khalesi 2014). Large replicated East Asian cohorts establish the mortality association (Kuriyama 2006; Abe 2019). Acute cognitive effects validated in repeated double-blind trials (Owen 2008). Cancer evidence is weaker and heterogeneous, but the cardiovascular and cognitive pillars are well-evidenced.
Replicated cohort signal across >300 000 Japanese adults: ~15–25% lower CVD and all-cause mortality at ≥5 cups/day vs none (Kuriyama 2006; Abe 2019). Effect is largest for stroke. Observational and East-Asian, but the mechanism (BP, LDL, vascular function) is supported by RCTs.
The caffeine + L-theanine combination measurably improves attention-switching accuracy and reduces self-reported tiredness in double-blind trials (Owen 2008; replicated by Giesbrecht 2010, Haskell 2008). The 'calm focus' subjective signature is real, distinct from caffeine alone, and arrives within an hour.
Small but real felt changes within weeks: the calm-alert pattern from L-theanine + caffeine arrives immediately, LDL fell 2.19 mg/dL and BP ~2 mmHg in pooled RCT data (Zheng 2011; Khalesi 2014) — modest in isolation, consistently positive.
Real but modest daily alertness lift driven mostly by the caffeine load (~25–45 mg/cup); the L-theanine attenuates the jitter without sedating. Anchors to acute Owen 2008 trial of attention and fatigue.
L-theanine increases alpha-band EEG power, lowers state anxiety, and reduces salivary cortisol response to acute stress in randomized trials (Hidese 2019). The effect is small and arrives blunted at dietary doses, but the calming side of caffeine's stimulation is genuinely present.
Indirect — better gum health and steadier vascular function support long-term facial appearance, but there is no direct skin or hair evidence at dietary doses. The effect is the side-shadow of internal health, not a meaningful cosmetic lever.
