The target is real; the pill is the argument. 99% of your calcium sits in bone. Eat too little for years and parathyroid hormone quietly pulls it back out to keep your blood level steady 1. You feel nothing. The bone thins anyway, and one day a curb you've stepped off a thousand times breaks a hip.
The strongest pill result came from frail nursing-home women, average age 84, starting from almost nothing: hip fractures dropped 43% 2. In healthy postmenopausal women the same supplement barely moved fracture risk 3, density gains ran 1 to 2% 4, and the U.S. Preventive Services Task Force now advises against it for prevention 5.
Same mineral, opposite effect, set by form. Calcium from food binds oxalate in your gut and lowers kidney-stone risk 6. A pill between meals binds nothing but still raises calcium in your urine, pushing stone risk up 7. And the heart-attack signal that won't quite die applies to supplements, never to dietary calcium 8.
Hit the number from food. Fill the gap only if one's left.
Mostly you feel nothing, and that's the point. If your intake was already fine, adequacy is invisible — you're maintaining a thing that wasn't broken. If it was low and you fix it inside a broader diet shift, blood pressure is the one early signal, down within weeks 11. The bone payoff never shows in a mirror. It shows up decades later as the hip that doesn't break at 78, and a walker you never needed.
The fine print — when to skip it, and what people get wrong
"More is better": the benefit flattens past 1000 mg; extra just raises urinary calcium 12. "Had a stone, cut calcium": backward — dietary calcium with meals lowers recurrence 6. "You need dairy": fortified plant milk, tofu, and greens hit the target without it.
After a heart attack or with high cardiac risk, don't start supplements without your cardiologist; food is unaffected. Skip between-meal pills if you've had a calcium-oxalate stone. Stage 4-5 kidney disease, thiazides, or high blood calcium override this — that's a doctor's call.
Common misses: a single 1000 mg pill (split it); carbonate on an empty stomach or a proton-pump inhibitor (switch to citrate 13); taking it beside levothyroxine, iron, or an antibiotic (stagger 2 to 4 hours); supplementing while vitamin-D-deficient 14.

Calcium
Store-brand carbonate runs ~$10-25/year; citrate ~$30-60/year. Hitting the target from food alone is also cheap — milk, yogurt, fortified plant milk, and canned sardines are commodity-priced staples.
Either a glass of milk, a yogurt, or two pills with meals daily. No willpower load, no lifestyle reorganization. The harder version is rotating food sources for variety, which is still trivial.
Multiple large RCTs including Chapuy 1992 (n=3,270) and the Women's Health Initiative (Jackson 2006, n=36,282), Lancet (Tang 2007) and BMJ (Bolland 2015; Tai 2015) meta-analyses, USPSTF 2018 guidance, and Cochrane reviews (Cormick 2015; Hofmeyr 2018). Mechanism is unambiguous. Holds back from 5 because effect heterogeneity between frail-elderly and community-dwelling populations is real and the cardiovascular signal is unresolved.
In frail elderly with low intake, calcium plus vitamin D cut hip fracture by 43% over 18 months (Chapuy 1992), and meta-analysis shows ~12% all-fracture reduction across populations (Tang 2007). Hip fracture carries 20-30% one-year mortality, so the effect is real, but the size in community-dwelling adults is small and contested (USPSTF 2018; Bolland 2015).
Maintained bone density at the hip and spine over decades preserves posture and the underlying skeletal frame of the face; the effect is marginal and slow, only legible when comparing trajectories at 70+ rather than in any short-term mirror check (IOM 2011; BHOF 2022).
Only legible when baseline intake was very low and a shift to a DASH-pattern diet drops systolic BP by ~5 mmHg in 8 weeks (Appel 1997). Most readers feel nothing within weeks; calcium is a silent maintenance nutrient, not a felt one.
