The old J-curve came from observational studies that lumped lifelong non-drinkers with people who'd quit because they got sick, making moderate drinkers look healthier than they were. Separate the two groups and most of it dissolves 1. Genetic-randomization studies pooling over 600,000 people then settled it: every level of drinking raised cardiovascular risk with the dose, and the best intake was zero 2 3.
The cancer link was never in doubt. Sufficient evidence covers seven sites, and breast cancer has no threshold — one drink a day raises a woman's risk by a few percent 4.
Sleep degrades from the first glass. REM drops from about two drinks and worsens with each one after 5; across five million wearable-tracked nights, one extra drink lifted overnight resting heart rate by two to three beats a minute 6. Going from one drink a day to two matched roughly ten years of brain aging on a scan of 37,000 adults 7.
Run a dry month — a controlled experiment with your own baseline as the control.
If you'd rather not stop, the targets have dropped below the old "moderate" line: on the order of one drink a day for women, two for men, with two alcohol-free days a week — and lower at every stop 8 9.
What changes when you stop.
- Week one: a few rough nights as REM rebounds, then overnight heart rate settles near your sober baseline and mornings sharpen.
- Month one: in the cleanest before-and-after trial of thirty days off, systolic blood pressure fell about 6 mmHg, weight about 1.5 kg, and liver enzymes normalized 10.
- Over years: brain volume preserved, cancer risk falling, more headroom in your fifties.
None of it is a movie ending. It's the slow tax, paid the other way.
The fine print — when to skip it, and what people get wrong
"Red wine is heart-healthy." The resveratrol case never replicated at drinkable doses, and the protective J-curve dissolves under causal analysis.
"A nightcap helps me sleep." It shortens sleep onset, which you feel, and suppresses REM and fragments the second half, which you don't. "I sleep better with wine" is reporting the falling-asleep, not the night 11.
"I don't flush, so I metabolize it fine." Fast clearance means the acetaldehyde passes through quickly — not that it never formed. The DNA damage doesn't care how briefly it was there 12.
Zero during pregnancy or while trying to conceive — the harm has dose-response from very low intake. Some drugs make alcohol dangerous: benzodiazepines and opioids (breathing), high-dose acetaminophen (liver), metronidazole and disulfiram (severe flushing). If you take any regularly, ask your prescriber.
