A cigarette burns tobacco at roughly 900 ยฐC and makes thousands of chemicals, including the tar, carbon monoxide, and carcinogens that cause the disease. A vape heats a nicotine liquid at about a fifth of that temperature and makes a fog you breathe in. No combustion, so almost none of the worst stuff 1. What stays: nicotine, which is addictive and raises heart rate, plus trace coil metals and flavour molecules nobody studied as things to inhale. So it isn't safe. It's far less of the worst, plus some new unknowns.
On quitting, the answer is settled. A nicotine vape beats the patches, gum, and lozenges the pharmacy sells, by a wide margin, and lands in the same bracket as prescription quit drugs 2. In the anchor trial, verified quit rates at a year were 18% for vapers versus 9.9% on nicotine replacement 3. Within weeks of fully switching, the lung-cancer-relevant tobacco chemicals in your urine fall toward never-smoker levels 4.
On long-term body harm, the answer is unwritten. Heart rate and airway irritation tick up, but far less than smoking 5. The modern vape is from 2007, so nobody has a thirty-year cohort yet.
Who it's for. If you smoke and want out, the trade is firmly in your favour: full switch, and the vape is the bridge 6. If you've never smoked, none of that benefit exists โ you'd be buying the unknown cost against nothing. Don't start 7.

- First week: taste and smell return; your clothes stop carrying the smell.
- A month or two: the morning cough fades and stairs stop being a workout.
- A year: exercise tolerance rebuilds and cardiovascular risk starts bending back toward baseline.
- A decade: most of the years cigarettes were going to take are back on the table, if you stay switched. The asterisk that never leaves: you're still on nicotine.
The fine print โ when to skip it, and what people get wrong
"As harmful as cigarettes": six in ten believe this; the biomarkers say otherwise 4. "Popcorn lung": real disease, never documented from vaping. "EVALI": traced to vitamin E acetate in illicit THC carts, not nicotine vapes 8.
Skip if pregnant โ nicotine in any form harms fetal development; ask a midwife. With unstable heart disease, prescription quit drugs are the safer route.
Dual use keeps the biomarkers near a smoker's. And four in five successful quitters were still vaping a year on โ coming off the vape is a second, harder project.

Heavy disposable use runs $30โ80 a month โ cheaper than cigarettes in high-tax places, on par in the U.S., more in low-tax countries.
For smoking cessation, this is one of the most strongly evidenced interventions in the catalogue. The long-term safety question is genuinely open โ no one has lived on a vape for thirty years yet.
Within a month of fully switching from cigarettes, the morning cough clears, taste and smell come back, and you stop smelling like an ashtray.
If you're a current smoker, fully switching captures most of the ten years cigarettes were going to take off your life. If you've never smoked, the math runs the other way.
Quitting cigarettes for a vape stops the long-run facial-aging trajectory of smoke. For a never-smoker, there's no skin gain on offer โ the win is the cigarettes you didn't keep smoking.
A small acute alertness bump from inhaled nicotine. Ex-smokers also get back the exercise capacity their lungs had been losing.
A mild attention lift in the minute or two after a puff. Once you're hooked, what feels like focus is mostly your dependence quieting back down.
A quick calming hit from inhaled nicotine. In a dependent user, that hit is what gets you back to baseline, not above it.
