Structural Nasal Obstruction
A nose that never quite works is one of the most ignored fixable problems in adult medicine, because almost nobody looks inside. Three structural causes cover nearly all chronic blockage: a bent septum, swollen turbinates, and nasal polyps. Most self-diagnose the septum and are wrong; nine in ten adults have some deviation, and few are bothered by it Mladina et al. 2008. The right move isn't surgery. It's a decision tree, and about half of cases never reach the scalpel.

For polyps the big shift was medical: injected biologics that target the underlying inflammation shrink polyps and bring smell back within four weeks 7. For everyday cases, daily steroid spray plus saline rinses give real relief that builds over weeks 8.

The order matters more than any single step. Work down the tree.

The timeline depends on which path you land on. Medical management pays off in 2 to 6 weeks: less stuffiness, dry-throat mornings gone, people stop calling you the one with the cold. Septoplasty gets worse for a week or two, then around six weeks the blocked side simply opens, and the gain holds at two years 6. Polyp treatment's headline is smell returning within four weeks and staying 7: people suddenly notice coffee, a partner's shampoo, dinner cooking. The fine print β€” when to skip it, and what people get wrong

New one-sided blockage with bleeding or facial numbness is a clinic visit this week; imaging rules out the rare cancer 5. If offered turbinate surgery, ask for mucosa-sparing technique; aggressive resection can cause the incurable empty nose syndrome 10.

Septoplasty isn't cosmetic; most insurers cover it as medical. Nasal surgery rarely cures sleep apnoea but does make CPAP tolerable 11. Polyps aren't just bad allergies; they come from a distinct inflammation, with smell loss as the tell 2.

Septoplasty fails about one time in five, usually a missed side-wall collapse or hidden turbinate problem 6. Polyps recur in up to 60% of severe cases within five years, so biologics treat them as chronic 12. Most "the spray didn't work" stories are technique stories.
