The pharynx behind your tongue is the one stretch of airway with no bone holding it open. Asleep its muscles relax, it narrows, and the air you pull in vibrates the loose tissue: the snore. Narrow a step further and the airway shuts, which is apnea 1.
Pure snoring is harmless to you; what it predicts is not. On overnight monitoring, moderate-or-worse apnea hit 14% of middle-aged men and 49.7% of men over 40 23. If you snore most nights in middle age, your odds of undiagnosed apnea are one in three to one in two. Worth resolving, not filing under "someday."
Untreated, the cost is one of the largest in this catalogue. Severe apnea roughly triples the death rate in the long cohorts 4 and nearly triples fatal cardiovascular events, while nightly CPAP users looked like people who never had it 5. Stroke risk roughly doubles and blood pressure climbs with the apnea count 67. Your partner loses an hour a night to it 8.

Two waves, if you have apnea. The first, within days to weeks: the afternoon wall lifts, the focus you'd written off as aging comes back, your partner sleeps. The second is the long one: the cardiovascular risk of treated patients looks like someone who never had apnea 5. For primary snorers, just a quieter bedroom within a week.

The fine print โ when to skip it, and what people get wrong
"Apnea is for fat older men" is half true: it also hits lean adults with a small jaw and women, who reach parity after menopause. Nasal strips are a comfort aid; if they help, your nose deserves a real evaluation.
Snoring gets missed when you sleep alone with nobody to report the pauses, or when a woman's apnea is read as insomnia. Treatment fails mostly from quitting CPAP in week one; a different mask or humidification usually fixes it.
