Bromelain has the most behind it. Added before scheduled dental or orthopaedic surgery, it fades the swelling a day or two faster 1; on any old random bruise it does much less 2. In acute sinusitis it clears symptoms about a day sooner 3.
Polyenzyme tablets (bromelain plus trypsin plus rutoside) matched diclofenac for knee-arthritis pain over three weeks, with fewer stomach complaints 4 5. Useful when NSAIDs are off the table.
Nattokinase nudges blood pressure down about 5 over 3 mmHg at 2000 fibrinolytic units a day, and thins the blood 6. No heart-attack-or-stroke trial exists; it's all intermediate markers 7.
Serrapeptase and papain are the weak end. Serrapeptase has old, small, industry-tied trials and a marketing story far past them 8; oral papain rides almost entirely on bromelain's data.
Match the enzyme to the job, and mind the empty stomach.
The fine print โ when to skip it, and what people get wrong
These thin your blood. Stop them at least 7 days before any surgery, extraction, or biopsy, and tell whoever's operating. Don't add them to a prescribed thinner (warfarin, apixaban, clopidogrel, cardiology-dose aspirin) on your own; nattokinase is not a substitute for a real anticoagulant. Avoid with a pineapple or papaya allergy, and in pregnancy.
"Take with meals for inflammation" is backwards and on most bottles; with food you get only the weak digestion effect. "Daily enzymes fix bloating" is mostly false 9. Serrapeptase does not dissolve fibroids, plaque, or scar tissue โ no controlled trial, no mechanism 8.
Why it "didn't work": taken with food when you wanted the systemic effect; an underdosed brand (two "500 mg" bottles can differ fourfold in active enzyme); uncoated serrapeptase, inert before the gut; or expecting a same-day hit, when the effect builds over 48 to 72 hours.

Around fifty to a hundred dollars a year for a decent brand; the cheap ones deliver almost nothing.
A capsule or two a day, on an empty stomach, for a one-to-four-week course around the event you're treating.
After a surgery, sprain, or sinus infection, the right enzyme on an empty stomach can shave a day or two off the swelling and the soreness โ small but real.
Several small trials back a few specific uses; the big independent studies that would settle the picture haven't been done.
Bromelain modestly speeds the fading of a fresh bruise โ useful around a dental extraction or a black eye, not an everyday cosmetic.
