Nasal Saline Irrigation
Pour warm salt water into one nostril, let it run out the other, two to five minutes at the sink. For chronic sinus trouble, hay fever, and a cold caught in the first two days, it is close to a free win: cheap, obvious, first-line in every ear-nose-throat guideline. In young children, starting salt drops early shortened colds by two days. One rule never bends, and here is what you didn't know you didn't know: plain tap water can kill you.

Your nose traps particles in mucus and sweeps them back with tiny hairs. A cold, pollen, or years of sinus disease jams it: mucus thickens, hairs slow, drainage paths swell shut. A high-volume pour, around 240 mL per nostril, washes the mucus and allergens out and rehydrates the surface so the hairs beat again. Salt stronger than your body's own pulls water from the swollen lining, so the passages open within minutes 1.

It works across three problems. For chronic rhinosinusitis, a daily rinse is first-line, alongside steroid sprays 2 3; in the clearest trial, rinsers cut their sinus medications and 93% kept it up afterward 4. For hay fever, less sneezing and less reaching for antihistamines 5. For a head cold, starting hypertonic rinses at the first symptom cut it short by about two days and reduced household spread 1; the 407-child ELVIS-Kids trial replicated it, with fewer pharmacy medications too 6. The trials are individually small; what moved the bar is mechanism, several trials, and clinic experience all pointing one way.

A squeeze bottle or a neti pot, not a spray. A drugstore spray puts five millilitres into the front of your nose; a rinse moves nearly fifty times that through the whole cavity and reaches the drainage paths a spray never touches 7.

If your nose was blocked, the first ten minutes are felt: the pressure eases and both sides open. Across the first week the morning throat-clear shrinks and nighttime mouth-breathing lets up; around two weeks in, the hay-fever antihistamine comes out less often 5. Over months, chronic sinus sufferers see fewer symptom days, fewer pharmacy trips, and the routine winter antibiotic course stops happening 4. The cold benefit is bounded: start in the first 48 hours or the trials say nothing 6.

The fine print β€” when to skip it, and what people get wrong

The water can kill you, and salt does not fix it. Tap water, even chlorinated, can carry Naegleria fowleri, an amoeba fatal in over 97% of cases once it reaches the brain; the salt packet does not sterilize it 8. Use only distilled, sterile, boiled-and-cooled, or 1-micron-filtered water. Headache, fever, or neck stiffness after a rinse is an emergency 9.

A saline spray is not a rinse β€” it moisturizes the front of the nose and is not what the trials test. A kitchen pitcher filter usually does not exclude the amoeba; check for a 1-micron absolute rating. Rinsing complements a steroid spray or antihistamines, it does not replace them 5.

If it stung, you used too much salt or skipped the baking soda; drop to isotonic. If nothing changed for a cold, you likely started too late. If you are immunocompromised, the distilled-only rule is absolute 10. Pregnancy is not a reason to skip it; rinses are recommended there 11.
