The one real use has a real mechanism. The cold-sore virus needs arginine, a different amino acid, to build new copies of itself. Lysine and arginine compete for the same route into your cells, so loading up on lysine nudges the ratio against the virus 1. It works clean in a dish and messier in a person.
The trials that found an effect all used a real dose. The positive ones ran 1 to 3 grams a day for months and saw fewer, milder outbreaks 2 3; one found 1.25 g worked and 624 mg didn't 4. The ones that found nothing used lower doses or shorter courses 5 6. Take enough for long enough and you have about a one-in-three chance of being a clear responder.
Two cycles in, you'll know. If the cluster you'd braced for didn't arrive, or a tingle came and never became a sore, keep going through the high-risk months. If your usual pattern rolled on unchanged, you're a non-responder; stop.
What being a responder actually buys you. Not a transformed life. The Tuesday tingle that doesn't become a Thursday sore. The photo you don't take with a hand half over your mouth.
You won't notice in the first month, because an outbreak you weren't going to have doesn't announce itself. Around the 8-to-12-week mark you look back and see the cluster you expected didn't come, or came smaller. And if it arrived on schedule, a $20 bottle told you this doesn't work on you.
If the cold sores are running your life, this is the wrong tool. Six or more outbreaks a year, large and painful, means asking your doctor about daily valacyclovir or acyclovir, which cut outbreak frequency by roughly 70 to 80% in trials, an effect lysine has never approached. If your protein is mostly rice, bread, and pasta, you may genuinely be short on lysine; the fix there is a daily portion of lentils, beans, tofu, or soy.
The fine print โ when to skip it, and what people get wrong
Collagen, skin, and bone claims outrun their evidence. The limiting step in collagen is enzymes, not lysine supply, and no trial shows supplements help skin, hair, or nails 7. The bone claim rides on one small urine study 8.
The immunity and calm stories only hold in the deficient. The calming effect showed up in villages living on wheat 9 or only when lysine was paired with arginine 10. A mixed diet puts you outside them.
"I tried it and it didn't work" is usually dose, timing, or diet. You took the 500 mg that found nothing; you started at the first tingle instead of daily; or you kept eating the nuts, seeds, and chocolate.
The honest fourth reason: about two-thirds of trial subjects saw little benefit even done right. The responder fraction is real but it isn't most people.
Skip it with any kidney trouble. The kidneys clear lysine, and a case report links years of 3 g/day to kidney failure; reduced eGFR makes this a doctor conversation. Skip it in pregnancy and breastfeeding too.
