An inguinal hernia is abdominal contents, usually fat or a loop of intestine, pushing through a weak spot in the lower belly wall. It makes a bulge that usually slides back when you lie down. The type decides the risk. An ordinary inguinal hernia in a man is the low-urgency kind. A femoral hernia sits just below, next to the leg vein, shows up mostly in women, and turns into an emergency far more often 1. Same lump in the mirror, different math.
Watching is a real option now, not neglect. Among 720 men with a mild hernia, pain was no worse in those who waited than in those who operated, with zero strangulations while they waited 2. Over the next eight years about two-thirds had the surgery anyway, because the bulge grew or the dragging got old, almost never as an emergency 3.
Waiting isn't free, though. Delay it a decade and your heart is a decade older going into the operation 4. And a femoral hernia flips the whole picture: roughly four in ten arrive as emergencies, a quarter of those needing bowel removed 5. Emergency repair kills about seven times as often as the planned kind 6.

If you repair it: mesh surgery is a day case, back to a desk in one to two weeks, full activity by six. The fix holds in 96โ99% of people 7. The change most people describe isn't feeling stronger; it's no longer checking for the bulge or bracing before they lift.
The fine print โ when to skip it, and what people get wrong
The catch worth weighing: about 10โ12% of people have lingering groin pain at a year, and in 1โ3% it's bad enough to regret the surgery 8, 9. Most settles by twelve months.
"Lifting caused it." A heavy lift reveals a weak spot; it doesn't create one. The real drivers are being male, family history, age, smoking, and a chronic cough or constipation 10.
"Any hernia will strangulate, so surgery's urgent." Not for ordinary inguinal hernias in men: under two trapping events per thousand patient-years, no strangulations in the trial 2. Femoral and women's hernias are the genuine exceptions.
Get to the ER now if a bulge turns constantly and severely painful, stops sliding back when you lie flat, or the skin over it goes red or dusky, worse with vomiting or fever. Trapped bowel can die within hours.
