Hair Transplants
A hair transplant moves follicles from the back of your head, where the balding hormone can't touch them, to the front, where they grow for life. At twelve months you get roughly the hairline you had at twenty-five. But the surgery does nothing to the surrounding hair, so the whole thing hinges on a cheap daily pill most discount clinics won't mention. Skip it, and in ten years you've bought a visible island floating on a scalp that kept receding.

Why it works: DHT, the hormone that shrinks your frontal follicles into invisibility, barely touches the ones at the back and sides. Move one to the front and it keeps growing the hair it always would have 1.

At a good clinic, 90โ€“95% of grafts take root and grow indefinitely 2. At the cut-rate end, survival drops to 75โ€“85%: a quarter of what you paid never comes in.

Strangers see it too. Shown the same men before and after, raters judge the after face as more attractive and successful, and patients' anxiety and depression scores fall measurably 3.

The pill is load-bearing. Finasteride 1 mg/day, started before surgery, kept 94% of patients visibly improved at a year versus 67% on placebo 4. The gap is the hair the placebo group lost from around the graft: the island forming.

The whole decision comes down to who holds the punch. A 3,000-graft FUE runs $2,000โ€“4,500 all-in in Turkey, ยฃ4,500โ€“12,500 in the UK, $6,000โ€“15,000 in the US. But price isn't the real variable: roughly one in five Istanbul clinics is unlicensed, with untrained staff working behind a listed surgeon's name 5.

The first six months are the price, not the payoff.

  • Weeks 1โ€“2: scabs at every graft site. Take the week off.
  • Weeks 2โ€“6: the shafts shed and you look thinner than before. This shock loss is normal, and it's when regret peaks.
  • Month 6: the fuzz is in, half the result. Month 12: the hairline the consult promised.
  • Years, if you kept the pill up: the surround holds, and the graft is indistinguishable, because it is your own hair.

Try the cheap version first. Finasteride plus topical minoxidil for a year halts progression in 80โ€“90% of users for under $400 4, which for many makes the surgery optional. Medication only holds what's still there, though; once the front is bare, surgery is the only way back.

The fine print โ€” when to skip it, and what people get wrong

Don't proceed if you smoke, have uncontrolled diabetes or hypertension, or have diffuse loss with no stable donor zone 6. Pregnancy rules out the finasteride stack. Untreated body dysmorphia guarantees a worse outcome, even from a perfect result.

"It's permanent": the grafts are; your other hair keeps receding without the pill. "More grafts is better": past ~4,500 per session, extraction damage climbs. "Turkey is dangerous, the West is safe": the axis is the surgeon, never the country.

Under 25 with an actively receding hairline: wait a year on finasteride and see where the pattern settles before operating. An overharvested donor and an aggressively low young hairline are both irreversible.
