The heart. In adults with prior heart attack, stroke, or clogged arteries, the shot cut major cardiovascular events by 20% over three years 1. Only about a third of that tracked with waist size dropping 2 โ the rest is the drug working directly on blood vessels, pressure, and inflammation.
The kidney. In diabetics with chronic kidney disease, major kidney events fell 24% and death from any cause fell 20% 3. It's the third drug class ever to bend the curve toward dialysis.
The airway. In moderate-to-severe sleep apnea, breathing interruptions dropped by 25 to 29 per hour, and about half the treated group fell to mild or no apnea 4. No pill or injection had ever moved these numbers.
The brain. A small trial of adults with alcohol use disorder drank less and craved less on a low dose 5. One study of 48 people โ promising, not proven.
Start with one thing: see the right specialist, not a weight-loss telehealth site.
Most of the win is a heart attack that never comes, a kidney still filtering โ things you never feel. What you do notice: nausea for the first weeks, then blood pressure and inflammation down at month three, and by month six the snoring gone or the stairs easier.
The fine print โ when to skip it, and what people get wrong
You stop and it comes back. Two-thirds of the lost weight returns within a year of stopping, with blood pressure and cholesterol drifting back too 6. Like statins, this is meant to be long-term.
Skip it if you or your family have medullary thyroid cancer, if you're pregnant or trying to be, or if you've had pancreatitis or severe gastroparesis. On insulin or a sulfonylurea, those doses come down to avoid a low.
"It's just for weight loss." The kidney and diabetes benefits show up where weight loss is modest. "It causes depression." Regulators reviewed it and asked to drop the warning โ no causal signal 7. "You can microdose for the brain effects." One 48-person trial; anyone selling that protocol is ahead of the data.

GLP-1s Beyond Weight Loss
Five large, separate trials in the last two years showed real benefits across heart, kidney, lung, and brain. As solid as drug evidence gets.
Blood pressure, blood sugar, inflammation, sleep apnea, heart failure symptoms โ all shift in the right direction within weeks to months.
Cuts heart attacks and strokes by about 20% in adults with heart disease. Slows kidney decline. Lowers all-cause death in CKD.
For moderate-severe obstructive sleep apnea: roughly halves nightly breathing pauses. About half of treated patients hit functional remission.
A weekly self-injection, plus a slow dose ramp up front. Build in strength training and protein to keep muscle.
Symptomatic heart-failure patients walk further; sleep-apnea patients stop crashing in the afternoon. Healthy users don't get a clear energy lift, and the first months can be nauseating.
Early evidence it blunts alcohol cravings and lowers substance use across the board. No clean effect on mood itself; older suicidality concerns have not held up under review.
$200โ$1,000+ per month, ongoing. Insurance may cover it for diabetes or heart disease; rarely for weight alone.
A slow body-composition shift over years; not why you'd take it. Rapid fat loss can age the face.
