Excess Sodium
You eat around 3,500 mg of sodium a day, and the shaker on your table is a tenth of it. Here's what you didn't know you didn't know: about 70% is already in the food before you buy it Harnack 2017, in the bread, deli meat, soup, and takeout. Pull intake toward 2,000 mg and the puffy face flattens inside a week, the cuff reads lower in a month, and you shed a load written into one in ten heart-disease deaths worldwide.

Salt pulls water. Eat it, and the body holds onto fluid to keep the concentration steady; plasma volume rises and blood pressure with it. In most people the kidneys clear the excess in a day or two. In the salt-sensitive (older adults, Black populations, anyone already hypertensive, people with extra weight, diabetes, or kidney trouble) that clearance lags, and the daily load hardens into chronic pressure 1.

Cutting it works, and few dietary exposures are better studied. Every gram you drop lowers systolic pressure about 1 mmHg, and roughly double that in people who already run high 2. The outcome data caught up: swapping regular salt for a 75/25 potassium blend in 21,000 adults cut strokes 14%, major cardiac events 13%, and deaths 12% over five years 3. Zoom out and about one in ten cardiovascular deaths worldwide trace to intake above 2,000 mg 4.

Three official ceilings exist and they disagree slightly: WHO says under 2,000 mg 5; the AHA and 2017 ACC/AHA guideline say under 2,300, with 1,500 ideal if you're older or hypertensive 6 7. Pick the lower number you can hold. Then, in order of yield:

One to two weeks: the face de-puffs, rings and sock-lines stop biting, the scale drops a pound or two of held water. If you run high, systolic falls two to five points in this window 2. Four to eight weeks: the full pressure effect lands, ankle swelling settles, older adults stop waking to pee at three in the morning, and the palate recalibrates. Years: the trials say fewer strokes, fewer heart attacks, fewer heart-failure hospitalisations 10 3. Stroke is the most sodium-sensitive of those, and the one most likely to end the life you have now.

The fine print โ€” when to skip it, and what people get wrong

Skip aggressive restriction if you have Addison's or other adrenal insufficiency, a salt-wasting kidney condition, POTS, or cystic fibrosis; endurance athletes and heat workers need more salt, not less. On antihypertensives, diuretics, or with kidney disease, change intake with your clinician so doses can track it.

The salt substitute is the exception: its potassium is unsafe in stage 3+ kidney disease, severe heart failure on potassium-sparing diuretics, or ACE-inhibitor plus ARB combinations โ€” groups the trial excluded 3.

"I don't add salt at the table, so I'm fine." The shaker is 5โ€“10% of intake; the rest is structural 8. "Sea salt is healthier." Same ~390 mg sodium per gram, and the trace minerals are negligible. "Only salt-sensitive people care." Sensitivity is a continuum; every adult's pressure responds 2.

"Low-sodium diets are dangerous, there's a J-curve." The PURE cohort showed one 11, but the 20-year randomised follow-up shows a straight line 12 and the outcome trial found benefit at every level 3. The signal is spot-urine bias and sick people eating less.

  • You cut the shaker and leave the bread, cheese, deli meat, and condiments. That's the 5% move without the 70% one; intake barely shifts.
  • You trade a packaged food for its "reduced sodium" version โ€” 25% less of a high number is still a quarter of the daily ceiling in one bowl.
  • You cook at home but lean on soy sauce, fish sauce, miso, bouillon, or parmesan, and push a "healthy" dinner past 2,000 mg.
  • You crash from 4,000 to 1,000 mg overnight and get light-headed; taper over a week or two instead.
