There are two versions, and they need different fixes. Sleep bruxism fires at night in bursts, off brief arousals out of deep sleep. Awake bruxism is a daytime habit: a held clench while you focus, drive, or scroll. Same worn teeth and sore jaw, different machinery.
The night version is not a habit. Each episode follows a small arousal surge, and the chewing muscles fire as part of that package 1. Sleep apnea is the most common trigger underneath: a rise in breathing effort precedes about half of all grinding bursts 2. The grinding is a flare from a fire downstairs.
A guard is armor, not an off-switch. Night guards reliably protect teeth from wear and modestly ease muscle pain, and do not reliably stop the grinding 3. Stopping the wear is the win. The muscles keep firing underneath, which is why scratches show up on the guard.
The daytime version runs on stress and, sometimes, your meds. SSRIs and SNRIs cause clenching as a dose-related side effect 4. Alcohol, caffeine, and nicotine each show dose-response links with night grinding 5.
The order is the point: find the driver, protect the teeth, then quiet the grinding.
What treating the driver buys you.
- Weeks: cut the alcohol and caffeine and the night grinding eases; a phone-prompt habit calms the daytime clench.
- Months: with apnea treated (usually CPAP), the arousals stop firing, the grinding quiets, and the afternoon fog lifts 7. Botox pain relief lands at 2 to 4 weeks 8.
- Years: the guard keeps the enamel, and the driver stays handled instead of quietly billing you for a decade.
The fine print β when to skip it, and what people get wrong
When the standard fix is wrong. If you snore or have witnessed breathing pauses, screen for apnea before a plain guard: a stabilization splint can worsen untreated apnea by pushing the jaw back 9. Use CPAP or a mandibular device.
"It's your bite." No. Bruxism is centrally driven; the bite plays a minor role, and grinding down healthy teeth to "balance" it is unjustified 10.
"It's just stress." True for daytime clenching. For the sleep version the link is weak, and "manage your stress" misses the airway 11.
The common misses. Treating the worn teeth without asking why, so undiagnosed apnea keeps damaging you behind the guard. Abandoning the guard because it doesn't stop the grinding. Ignoring a child's grinding, which can flag obstructed nighttime breathing.
