The lever is your exhale. Slow breathing turns up the parasympathetic dial through the vagus nerve within seconds, and lengthening the exhale relative to the inhale amplifies the effect 1. Around six breaths a minute, heart rate and breathing lock into rhythm and heart-rate variability multiplies 2. The sigh has its own dedicated wiring: a cluster of brainstem neurons that trigger sighs projects straight into the brain's main alarm system, so a deliberate double-inhale-plus-long-exhale is close to a hard-wired reset button 3.
The effect is real and quick. Cyclic sighing produced the largest mood gain and the steepest drop in resting breathing rate, with more practice giving more benefit 4. Slow breathing at roughly six breaths a minute has large effect sizes for anxiety across more than eighty pooled trials 5, 6. On blood pressure it's quieter: two months of it dropped systolic pressure by about 7 mmHg in hypertensives 7, roughly 3.7 mmHg pooled 8 โ the range of a low-dose drug, though some is expectation.
Underneath, they're one move: belly-led breathing with the timing changed. Pick the pattern by the job.
How it lands.
- First session: hands warm, shoulders drop, the loud channel quiets inside three minutes. Your heart rate is measurably falling 9.
- First month: faster to sleep the nights you do 4-7-8, a notch calmer walking into the meeting you dread, resting breathing rate a little lower 4.
- First year: if you're hypertensive and stuck with it most days, systolic pressure down by 4 to 7 mmHg 7. And arousal starts to feel like something you can move, not something that just happens.
The fine print โ when to skip it, and what people get wrong
Why nothing happened. You only practiced mid-panic (train it calm, so the brake works under load); you used box breathing to fight a spike (it's for composure, not fast calm-down); or you were quietly hyperventilating โ mouth-only inhales or an exhale shorter than the inhale. Aim for less air with a longer exhale, breathing in through the nose.
Any pattern works. No โ in the trial, cyclic sighing beat box breathing on mood at the same dose 4. The Navy SEALs use box, so it's best. Operational use isn't trial evidence; long exhales calm faster. 4-7-8 knocks you out in 60 seconds. Real mechanism, rhetorical timing. Breathe deeply all day. Chronic over-breathing raises anxiety instead of lowering it.
When to skip the holds. With uncontrolled high blood pressure, heart-rhythm disorders, pregnancy, seizure or panic disorder, or recent eye or abdominal surgery, drop the breath-hold patterns (box, 4-7-8) and forced-hyperventilation styles; slow paced breathing without holds is usually fine. Light-headedness on early 4-7-8 attempts is common and harmless โ cut the cycle count.
