Anything labelled wrinkle-free, non-iron, permanent-press, or easy-care was cross-linked with a formaldehyde-based resin to lock its shape 1. The resin slowly breaks down against your skin, releasing free formaldehyde. Cotton takes the cross-linker best, so a pure-cotton non-iron shirt is the highest-residue thing in a wardrobe.
New polyester, nylon, and acetate carry disperse dyes that never bond to the fibre; sweat and friction draw them back out onto skin 2. That is why a new black t-shirt rinses dark, and why new denim leaves indigo on your leg.
A single machine wash removes about 60% of the free formaldehyde on the first cycle 3; loose dye comes out on the same schedule. For most people that is enough to notice nothing.
Priority order: non-iron shirts first, then new dark synthetics, then new sheets and pyjamas (eight hours of skin contact a night). A plain undyed cotton t-shirt is the one case where skipping genuinely costs nothing. An Oeko-Tex hangtag means the item was tested for low residual formaldehyde and banned dyes, so its starting load is already much lower.
Skip it and most people get the half-noticed itch that fades within a week. But for the slice whose immune system has learned to react (one in twenty to one in seven of dermatology-clinic patients, fewer in the public), it becomes a rash where fabric presses under heat, on the neck, the inside of the elbows, the waistband 1. It looks like ordinary eczema, so people blame detergent or stress for years before anyone names the shirt 5.
The fine print — when to skip it, and what people get wrong
The parasite headline is overstated. Try-on scabies is rare; the mites survive barely a day off skin and need longer contact than a fitting takes 4. The real reason to wash is the dermatitis.
"Natural fibres are safe" is backwards for formaldehyde. Cotton is the most heavily treated fibre 1; "natural" on a label tells you nothing about residue, while "untreated, undyed, Oeko-Tex" does.
A cold, short cycle on a non-iron shirt. It pulls out less of the highest residue load you own, so the low-grade neck rash still shows and gets blamed on anything but the shirt 6.
"I'll do it next time." Drop the bag next to the washer on the way in, not in the wardrobe.

Washing New Clothes Before Wearing
A single extra wash cycle before first wear, performed as needed when a new garment enters the wardrobe. Activation energy is psychological (treating new as not-yet-ready), not logistical.
Patch-testing prevalence data are solid (Lazarov 2004, Hatch and Maibach 2000, Reich and Warshaw 2010); GAO 2010 content testing of US-retail garments is documented; washing-reduction figures (~60% formaldehyde removal in a single wash) come from industry and government testing. No RCT directly compares wash-vs-no-wash dermatitis incidence in unselected populations — mechanism plus residue-measurement chain, not outcome trials.
Pre-wear washing prevents first-wear irritant rashes and a fraction of frank allergic contact dermatitis cases. Patch-test cohorts show 1–16% of textile-dermatitis-suspected patients react to disperse dyes; 18–21% to formaldehyde-releasing resins (Lazarov 2004). For unsensitised adults the win is small; for sensitised individuals and infants it is real and named.
Loose disperse dyes and indigo can rub off onto skin from unwashed new garments; washing removes the transferable fraction. Effect is subtle — visible mainly on light skin from saturated synthetic colours — and is the weakest of the entry's claims.
