Every fibre is competing on one thing: where the sweat goes. Cotton absorbs it and holds it, sitting wet against skin until air dries it. Polyester and nylon refuse to absorb and shuttle sweat to the outer face to evaporate. Both work in their home environment and fail outside it: cotton is kind to an open vulva under loose clothing, brutal inside a closed shoe; synthetic is great in a ventilated shirt, bad in tight underwear where the sweat has nowhere to go. Merino cheats the rule, storing moisture in the fibre core while the skin stays dry-feeling.
Wet skin is where every downstream problem starts. Skin softened by hours of moisture has two to three times the friction of dry skin, and it's the substrate athlete's foot and yeast need to grow. Runners wearing cotton on one foot and acrylic on the other got twice as many blisters, three times the size 1. In closed boots a merino sock kept skin drier than polypropylene 2. The bacteria that turn sweat into B.O. grow almost exclusively on polyester, barely on cotton 3. Below the waist the trials are thinner, but the same heat-and-moisture model backs cotton for recurrent yeast 4.
The rule splits at the waist.
A shortcut: can air reach the fabric? Under jeans in a chair, no, so use a fibre that absorbs (cotton, wool). A mesh running shoe, yes, so synthetic wicking works.
For three groups this stops being a comfort tweak and becomes a real intervention. Women getting more than two yeast infections a year: swap every synthetic pair for cotton, and sleep without underwear when possible. People with sweaty feet: merino, a midday sock change, and two pairs of shoes rotated so each dries. Diabetics: the guidance is synthetic or wool over cotton, because moisture-softened skin on a foot with reduced sensation is how ulcers start 5. This is the strongest indication in the entry.
The fine print — when to skip it, and what people get wrong
"Bamboo is naturally antibacterial." It's rayon — the FTC fined major retailers for the claim, since the chemical processing destroys the plant's antimicrobial compounds 6. Fine underwear, not a wellness product.
"Polyester underwear makes you infertile." That comes from men wearing a tight polyester scrotal sling for a year 7. In normal wear, fit raises scrotal temperature; the fibre does not.
The cotton sock that stays on from commute through workout to the bus home. The fibre isn't the whole problem; the unchanged wet sock is. Switch to merino for athletics and change right after.
Merino washed hot with bleach. Cold water, wool detergent, no softener — heat and alkaline strip the surface chemistry its odor advantage rests on.

Underwear and Sock Fabric
Cotton underwear is cheap; merino socks cost more per pair (~$15–25 each) but last longer than cotton equivalents. Incremental annual cost over generic cotton everything is roughly $30, well within the trivial-cost band.
One-time drawer audit and replenishment; no ongoing daily action beyond what people already do (get dressed). Some merino-care friction (cool wash, no bleach) is real but minor.
Multiple small RCTs (Herring & Richie 1990; Bogerd et al. 2011/2012; Tanaka et al. 2013) plus aligned major-society guidance (ADA, ACOG, CDC) show clear functional gains in comfort, friction-blister incidence, and fungal/bacterial flare rates. The effect is largest in symptomatic subgroups but felt within a week or two by most users who switch.
Multiple small RCTs in military and athletic populations (Herring & Richie, Bogerd, Knapik), one good placebo-controlled trial in tinea pedis (Tanaka et al. 2013), a strong odor lab study (Callewaert et al. 2014), and consistent ACOG/CDC/ADA practice guidance. No large outcome RCT in general-population everyday wear, which keeps it below evidence=4.
Sock-borne body odor is a near-pure function of fibre choice — Callewaert et al. 2014 showed Micrococcus enrichment on polyester drives the smell, and merino wool produces less foot and axillary odor across multiple panel studies (McQueen et al. 2007). For people who notice their feet smell, swapping cotton/synthetic socks for merino is a directly visible (or rather, smellable) win within days.
Preventing recurrent tinea pedis, tinea cruris, and chronic friction-related skin changes (lichenification, dyschromia from repeated dermatitis) yields a small cumulative aesthetic benefit on visible foot and groin skin over years. Real but modest; this is not a face-skin intervention.
Comfortable, dry feet and dry underwear remove a small but real daily drag — less chafing, less itching, less self-conscious adjustment. Trivial positive.
Reduced foot odor and reduced recurrence of yeast infections lift a real source of low-grade self-consciousness and discomfort. Small contribution to daily wellbeing.
