The window is the whole story. For roughly the first twelve weeks the baby has no working thyroid; every molecule of thyroid hormone in its developing brain crosses the placenta from you. The fetal gland isn't independent until about week 18–20 1. Pregnancy also loads your own thyroid hard, and a gland already on partial reserve tips over silently.
The numbers shift, too. Pregnancy pushes TSH down, so a 3.5 that looks fine on a routine report is borderline-high, and a 0.2 that would mean overactive thyroid otherwise is normal at twelve weeks 2. The range most labs print is not a pregnancy range.
Children of mothers whose hypothyroidism went undiagnosed in pregnancy averaged seven IQ points lower at ages seven to nine, and four times as many scored at or below 85 — 19% versus 5% 3. Untreated hypothyroidism also roughly doubles the miscarriage risk and raises preterm birth 4. Thyroid antibodies matter on their own: about one woman in ten carries them, and even with normal hormone numbers they carry roughly three times the miscarriage rate and twice the preterm rate 5.
The whole move is getting one test ordered early — not at the routine 10–12 week visit.
What catching it early buys you. If overt hypothyroidism is found and treated, the felt change over the first four to eight weeks on levothyroxine is usually plain: the tiredness lifts, the fog clears, the low mood eases. The bigger payoff is the part you never see — the miscarriage that doesn't happen, the seven-point IQ gap that doesn't open in your child 3. Postpartum, a single TSH at three and six months catches the thyroiditis routinely mistaken for depression 8.
The fine print — when to skip it, and what people get wrong
"My TSH was normal, the lab said so." The printed range is for non-pregnant adults; a 3.8 marked "within normal limits" is borderline-high in pregnancy 2.
"I'm not high-risk, so I don't need it." Testing only high-risk women misses roughly a third of cases 9. The blood draw is cheap; the harm it prevents isn't.
"Postpartum I'm just exhausted — it's depression." Postpartum thyroiditis hits 5–10% of women and looks identical; the standard screen skips TSH 8.
Waiting for the 10-week appointment. The leverage is at six weeks; request the TSH order by phone or portal when the home test is positive.
Treatment started too late. Two trials starting at 13–17 weeks found no IQ benefit, so a borderline number found at twenty weeks isn't worth self-blame 10 11.

Thyroid Disease in Pregnancy
TSH ~$20–60 cash, generic levothyroxine ~$30–60/year, both typically insurance-covered. Trivial against the intervention's stakes.
Daily levothyroxine on an empty stomach 30–60 minutes before food, separated from prenatal vitamins, plus TSH retests every 4 weeks through mid-pregnancy. Sustained low-grade discipline, not a major reorganization.
Overt-disease evidence is settled: Haddow 1999 cohort IQ data, ACOG and ATA guideline consensus (Haddow et al. 1999; Alexander et al. 2017). Two large RCTs cleanly negative for treating subclinical disease at mid-pregnancy (Lazarus et al. 2012; Casey et al. 2017). TABLET clean negative for treating euthyroid TPO+ (Dhillon-Smith et al. 2019).
Treated overt hypothyroidism resolves fatigue, brain fog, cold intolerance, constipation, and low mood within 4–8 weeks — clear functional improvement (Alexander et al. 2017); postpartum thyroiditis detection prevents months of mis-treated mood/fatigue symptoms (Stagnaro-Green 2012).
Fatigue is the single most reported symptom in maternal hypothyroidism; correction with levothyroxine produces clear, daily-felt energy lift within weeks (ACOG 2020). Postpartum thyroiditis fatigue is the textbook misdiagnosed-as-PPD presentation (Stagnaro-Green 2012).
Maternal hypothyroidism drives depression and anxiety during pregnancy; hyperthyroidism drives anxiety; postpartum thyroiditis is the most common organic cause of postpartum mood disturbance misdiagnosed as PPD (Stagnaro-Green 2012). Treatment effect on mood is consistently observed clinically.
Brain fog improves with TSH normalization in overt hypothyroidism; real but secondary to the energy/mood lift (Alexander et al. 2017).
For the mother, catching previously-undiagnosed Hashimoto's via pregnancy screening enables lifelong management; the dominant longevity effect is on offspring (~7-point IQ deficit in untreated overt hypothyroid mothers' children, Haddow et al. 1999), which is one-generation-out from a maternal longevity axis.
