One process, four pains. Relaxin makes your ligaments stretchier from the first trimester so the pelvis can open 1. That same slack, plus a uterus that gains around twentyfold in weight, produces the lot. The round ligaments, cords anchoring the uterus, get tugged when you move suddenly, giving a sharp one-sided groin jab that's harmless to the baby 2. The pelvic joints loosen and slide, giving a deep grinding pelvic girdle pain at the pubic bone and buttocks 3. The forward belly-weight overloads the low back and flares the lower ribs.
The pain is treatable, and cheaply. Structured exercise lowers pregnancy back pain and drops pain roughly a point on a 10-point scale 4 5. A $20–50 belt worn low across the hips cuts sacroiliac movement and walking pain the day you put it on 6. For severe pelvic girdle pain, a taught programme of deep-core, glute, and pelvic-floor exercises beats generic physio and holds up two years out 7. Each is modest alone; stacked, they pay out across pain, sleep, and mood at once.

The arc.
- Days: the pillow stack and log-roll habit stop most night pain and round ligament jabs; the belt earns its keep the first walk to the shop.
- Weeks: exercise compounds — a point off through movement, another through a physio's stabilising programme 4.
- A year on: you're far likelier to skip the roughly one-in-five who still hurt when the baby turns one 11.
Treat the protocol as load-bearing from early on if you had pelvic girdle pain last pregnancy (recurrence 65–85%), have prior back pain or hypermobility, a higher BMI, physically demanding work, or a twin pregnancy 14 15.
The fine print — when to skip it, and what people get wrong
Musculoskeletal pain changes with how you move. Pain that doesn't, that stays constant and unrelieved, isn't round ligament. Same-day call your provider for constant abdominal pain, any bleeding, fluid leak, or contractions before 37 weeks, fever, severe headache or upper-belly pain (preeclampsia), one-sided calf swelling (clot), or reduced fetal movement.
"Just tough it out" is the expensive myth: untreated severe pelvic girdle pain roughly doubles perinatal depression risk 12. Bed rest is the wrong fix; it deconditions the muscles and raises clot risk 3. Acetaminophen doesn't cause autism once family confounding is accounted for 13.
