The bacteria arrive on the seed, picked up months earlier in the field, often a continent away. The germination tank does the rest, turning a handful of cells into millions by the time the sprouts hit the clamshell 1. And one lot feeds dozens of growers: a single 1999 batch produced 157 Salmonella cases across ten states 1.
They also work into the seed coat, past any surface wash. The industrial chlorine soak cuts the load about a thousandfold but is too variable to trust, which the FDA treats as the ceiling of the current toolkit 2 3.
This is a settled, thirty-year pattern: 52 sprout outbreaks from 1996 to 2020, more than 2,700 illnesses, 179 hospitalizations, and five deaths 4 5. Two events show what bad looks like. Sakai, Japan in 1996 sickened 6,309 schoolchildren and killed three 6. Germany in 2011 caused 845 cases of kidney-shutdown syndrome, the largest cluster ever recorded, and 54 deaths, hitting healthy adult women hardest 7.
The point isn't that you'll get sick. Most readers won't. It's the shape of the risk. When the lot is bad, the toxin can destroy red blood cells and shut the kidneys down, and adults over sixty die at the highest rate 7. For a pregnant reader the pathogen is Listeria, and the asymmetry is sharper: roughly ten times the susceptibility, and one in four placental infections end in miscarriage, stillbirth, or a newborn in intensive care 8. The bad lot lands on the people least able to afford it.
The advice is binary. No safe-sourcing trick helps, because the contamination is on the seed itself.
Want the crunch? Anything thin and crisp does the same job on a sandwich: shaved cabbage, julienned carrot or radish, shredded lettuce, cucumber ribbons. For nutrition, any leafy green matches raw sprouts per gram. The one real outlier is broccoli sprouts and their sulforaphane, which you can get by steaming mature broccoli.
Sprouted-grain bread and microgreens are different foods, with the wet phase short or over, and don't carry the same risk.
The fine print — when to skip it, and what people get wrong
Organic and local aren't safer: the 2011 German outbreak was certified-organic seed. Home-growing isn't safer: same seed, same warm-wet phase, no industrial disinfection step. Rinsing doesn't fix it: the bacteria sit inside the seed coat, where water can't reach 2.
If you are pregnant, over 65, immune-suppressed by chemotherapy, biologics, or transplant drugs, or feeding a young child, skip raw and lightly cooked sprouts entirely — alfalfa, clover, radish, mung, and onion alike 5 8.

Three decades of consistent outbreak data: FDA documented 52 sprout-associated outbreaks 1996–2020 with 2,700+ illnesses (FDA 2022); the Sakai 1996 white-radish-sprout outbreak hit 6,309 schoolchildren (Michino et al. AJE 1999); the 2011 German fenugreek-sprout E. coli O104:H4 outbreak produced 3,816 infections and the largest HUS cluster ever recorded (Buchholz et al. NEJM 2011). FDA and CDC agree; mechanism (warm-humid germination amplifies seed-internalized pathogens) is well established (Taormina et al. EID 1999; Ding et al. J Food Sci 2013).
Avoiding raw sprouts removes a recurring vehicle for Salmonella, Shiga-toxin E. coli, and Listeria infection; CDC's National Outbreak Reporting System recorded 1,498 sprout-linked illnesses and 179 hospitalizations 2000–2020 (Salmonella alfalfa-sprout clusters in 2009, 2014, 2016 and chronic clover-sprout E. coli outbreaks at one major chain through the 2010s being the U.S. anchor cases). For the typical reader the per-meal risk is low; the score reflects that the win is a real but small reduction in acute-illness probability.
Tail-shaped mortality avoidance: 2–7% of E. coli O157:H7 / O104:H4 infections progress to hemolytic uremic syndrome with case-fatality 1–4% acutely and ~5% of survivors progressing to end-stage renal disease (Frank et al. NEJM 2011; the 2011 German fenugreek-sprout outbreak produced 845 HUS cases and 54 deaths). Pregnancy adds Listeria with ~25% fetal mortality once invasive (FDA 2024). Population mean effect is small; the worst-case is catastrophic.
