A normal adult cycle is 24 to 38 days, bleeding eight days or fewer 1. The 28-day textbook number is a population average, not a target; real cycles average about 29 days and vary widely, month to month 2. Ovulation can land anywhere from cycle day 8 to day 25 in a cycle that looks perfectly regular 3.
The cycle is upstream of the labs. Chronically long or irregular cycles carry roughly twice the type 2 diabetes risk, independent of weight 4, and 30 to 40% higher risk of premature death, mostly cardiovascular 5. Fasting glucose flags it only years later.
For severe premenstrual mood and heavy bleeding, the log is the diagnostic instrument. Diagnosing PMDD requires two cycles of daily ratings, because memory is too biased by today's mood 6. To time conception or avoid pregnancy without hormones, add a real ovulation signal: morning basal temperature (a $10 thermometer catches the 0.3 °C rise), cervical fluid, or LH strips. For contraception, combine two; one alone is calendar-grade and five times weaker 10.
How it pays off, over time.
- Cycle three: a baseline. Your "irregular" periods are either that or inside the normal band, and the mood crash either lines up with the luteal phase or it doesn't 1.
- Six months: the heavy-bleeding question is settled with a number a clinician acts on 7, and any fertile window has stabilised 3.
- One year: the appointment is a graph, holding the two cycles a diagnosis needs 6.
- Five years: a long irregular run, the first sign of insulin resistance, is visible in a way memory never made it 4.
The fine print — when to skip it, and what people get wrong
Fertility awareness is the wrong sole contraception if pregnancy would be a disaster: typical-use failure is about 7 in 100 a year 11 versus 1 in 200 for an IUD 12. On hormonal birth control, postpartum, or breastfeeding, the signal is unreliable. Where abortion is criminalised, cycle data has been subpoenaed; use a local-only or encrypted app.
"Ovulation is day 14" assumes a textbook 28-day cycle; real ones ovulate day 8 to 25 3. Calendar-only apps just subtract 14 from an average, wrong by around 25%, so timing needs a real biomarker 10.
An empty dashboard reads "regular" when cycles have actually stopped from undereating, overtraining, or stress; three months of silence is a workup and a warning of bone and fertility loss 13. A log that never reaches a clinician captures the signal without acting on it 9.
