MCV და RDW — ანემიის გასაღები
Your doctor read a low hemoglobin, said "borderline anemia, eat more iron," and sent you home. That guess is wrong about half the time, and the two numbers that name the real cause were already on the same page. MCV is how big your red cells are; RDW is how mixed in size they are. Read together they sort a low count into one cause and one confirming test: iron, B12, folate, an inherited trait, or the marrow itself.

The two numbers see different things. MCV is average cell size in fL, normal 80 to 100. Cells run small when the marrow is short on iron; they run big when it can't build DNA fast enough, as in B12 and folate deficiency, alcohol, and a few drugs 1. RDW is how much the cells vary, normal about 11.5 to 14.5%. It is the sensitive one and moves first: in iron deficiency it rises weeks before MCV ever drops below normal 2. MCV tells you the kind of trouble; RDW tells you it started.

Pull up your most recent test, find MCV and RDW, use the reference range printed beside each value, and read across.

When the grid is ambiguous, the blood smear is the tiebreaker 6.

Three findings you cannot afford to miss. Iron deficiency, because oral iron lifts your energy within a week 2. B12 deficiency, because tingling fingers are early nerve damage that stops fully reversing past six to twelve months untreated 7. And past seventy, a high MCV with a high RDW can be myelodysplastic syndrome, a treatable marrow disease that gets blamed on age instead 5.

What repletion feels like. Iron: within a week the stairs stop pounding your chest, by week three the household stops catching you asleep after dinner, and the restless legs fade as stores refill 2. B12: the marrow floods out new cells in three to seven days, brain fog clears over a couple of weeks, and the tingling pulls back as far as the damage let it 7. Thalassemia trait: nothing changes, except no one prescribes you iron you don't need again.

The fine print — when to skip it, and what people get wrong

A normal MCV doesn't rule out deficiency: iron and B12 cancel at the average, leaving RDW the only abnormal number 8. A high MCV isn't automatically B12 either; a third to a half is alcohol, liver, thyroid, or a drug 1.

The grid lies for 120 days after a transfusion, during active repletion, and in inflammation, which lifts ferritin above its cutoff on empty stores 2. On dialysis or with chronic liver disease, ask for a hematology consult.
