Everyone knows these as weight-loss shots. Here's what you didn't know you didn't know: the same weekly injection cuts heart attacks and strokes by about a fifth in people with existing heart disease, and most of that protection has nothing to do with the weight you lose. It slows failing kidneys, halves nightly breathing pauses in sleep apnea, and quiets the urge to drink. One molecule, in one body, moving the heart, kidney, airway, and brain at once — and if any of those is your problem, that changes the conversation with your doctor.

The heart. In adults with prior heart attack, stroke, or clogged arteries, the shot cut major cardiovascular events by 20% over three years 1. Only about a third of that tracked with waist size dropping 2 — the rest is the drug working directly on blood vessels, pressure, and inflammation.

The kidney. In diabetics with chronic kidney disease, major kidney events fell 24% and death from any cause fell 20% 3. It's the third drug class ever to bend the curve toward dialysis.

The airway. In moderate-to-severe sleep apnea, breathing interruptions dropped by 25 to 29 per hour, and about half the treated group fell to mild or no apnea 4. No pill or injection had ever moved these numbers.

The brain. A small trial of adults with alcohol use disorder drank less and craved less on a low dose 5. One study of 48 people — promising, not proven.

Most of the win is a heart attack that never comes, a kidney still filtering — things you never feel. What you do notice: nausea for the first weeks, then blood pressure and inflammation down at month three, and by month six the snoring gone or the stairs easier.

The fine print — when to skip it, and what people get wrong

You stop and it comes back. Two-thirds of the lost weight returns within a year of stopping, with blood pressure and cholesterol drifting back too 6. Like statins, this is meant to be long-term.

Skip it if you or your family have medullary thyroid cancer, if you're pregnant or trying to be, or if you've had pancreatitis or severe gastroparesis. On insulin or a sulfonylurea, those doses come down to avoid a low.

"It's just for weight loss." The kidney and diabetes benefits show up where weight loss is modest. "It causes depression." Regulators reviewed it and asked to drop the warning — no causal signal 7. "You can microdose for the brain effects." One 48-person trial; anyone selling that protocol is ahead of the data.
