The calcium is scar tissue, not the disease. Plaque starts as soft, cholesterol-rich deposits in the artery wall; over years the body lays calcium into the mature ones like scar over a wound. The Agatston score adds up those calcified deposits. Any number above zero means atherosclerosis is present now, not "might develop," and the bigger the number, the more disease. Two people with identical cholesterol can score 0 and 600. They are not the same patient.
It's the test for the "we could try a statin, up to you" conversation. Adding the score to a standard risk calculator changes the answer for about a third of intermediate-risk adults — half move to lower risk, half to higher 1. Above zero, the hazard for a cardiac event runs roughly four to seven times higher, and the pattern holds across every ethnic group tested 2. A zero holds annual mortality under 1% for fifteen years 3. A score above 1,000 puts you in the same bracket as someone with a documented heart attack 4.
Ask for it if you're 40–75, have a risk factor, and the statin question is genuinely open.
Reading it: 0 means no hardened plaque and, for most, no statin needed. 1–99 is mild; 100–299 makes a statin clearly indicated. 300–999 is extensive. 1,000 or above is treated like established heart disease — high-intensity statin, aspirin, aggressive blood pressure control 6. Read the absolute number rather than the age percentile.
A zero mostly changes what's in your head. The low background hum of "should I be doing something about my heart" goes quiet, and the answer holds for years 3. A positive score changes what you take: a first statin conversation, a $4-a-month pill at breakfast, a home blood-pressure cuff, a lipid panel every six months for the first year. Over a decade the payoff is real. A statin cuts heart-attack risk by about a third, concentrated in exactly the people the score flags as having plaque to begin with 1.
The fine print — when to skip it, and what people get wrong
A statin raises your calcium score. It converts soft plaque toward the stable calcified type, so repeat scans can't judge whether the drug works 6. And a zero isn't permanent: today's zero doesn't lock in a zero a decade out.
Under 45, a zero can lie. Early plaque is soft and uncalcified, so a young adult with high Lp(a) can score zero while disease builds. Diabetics get a shorter warranty, five to ten years 7. For those profiles, ApoB or CT angiography carry more signal.

Coronary Artery Calcium (CAC) Score

One ~15-minute appointment, no contrast, no fasting, no prep. Total reader effort is finding a center and showing up once. Result-driven follow-up conversation with a clinician adds ~30 minutes downstream.
Strong indirect longevity effect via guideline-directed therapy escalation in high-CAC patients and appropriate deferral in CAC=0 patients. MESA hazard ratios for incident coronary events 3.9x for CAC 1-100, 6.8-7.1x for CAC >100 versus zero (Detrano 2008); CAC >=1000 carries event risk similar to secondary prevention (Peng 2020). CAC-guided statin allocation identifies the high-burden subgroup that benefits most from intensive LDL lowering (Nasir 2015, Orringer 2021).
Multiple large prospective cohorts (MESA n=6,722, Heinz Nixdorf Recall, Dallas Heart, CAC Consortium n=66,636) consistently show graded event prediction independent of traditional risk factors (Detrano 2008, McClelland 2015, Peng 2020). Class IIa recommendation in 2018 AHA/ACC cholesterol guideline and 2019 ACC/AHA primary prevention guideline (Grundy 2019, Arnett 2019). Held back from 5 by the absence of a completed RCT of CAC-guided care versus standard care — the USPSTF's reason for an I-statement (USPSTF 2018).
Acting on the result — typically initiating or deferring statin therapy — is what delivers the benefit. The test itself produces no felt change. The small short-term win is decision clarity: an evidence-based answer to 'do I need a statin?', often months of guesswork compressed into one number (Grundy 2019, Nasir 2015).
Two-sided effect on inner wellbeing: CAC=0 in an asymptomatic adult removes a meaningful background worry, with a documented ~15-year warranty period of <1% annual mortality (Valenti 2015, Mortensen 2022); a high score can spike health anxiety until the treatment plan is in place. Net positive for most patients, but the magnitude is modest and depends on result.
