The sprout stores a compound called glucoraphanin and, in separate cells, an enzyme called myrosinase. Chewing crushes both together, and the enzyme cuts glucoraphanin into sulforaphane — the most potent natural switch known for Nrf2, the circuit that fires up your body's toxin-clearing enzymes 1. Three-day sprouts pack 10 to 100 times more glucoraphanin per gram than a mature broccoli head 2.
The clearance win is the settled part. In a 12-week trial of 291 adults in a high-pollution region of China, a daily sprout drink raised urinary excretion of benzene byproducts 61% and acrolein byproducts 23% from day one, with no fade 3.
In people who already run high, sprouts also move the numbers a doctor watches: systolic blood pressure down about 11 points and diastolic about 7 points over four to twelve weeks 4. In type 2 diabetes, sprout powder cut fasting insulin and insulin resistance 5 and lowered blood inflammation markers 6. Healthy normotensives see less.
Half a cup of raw sprouts a day, chewed well. That is roughly 40 to 50 grams, the 50 to 100 micromoles of sulforaphane the working trials used. Chewing is what triggers the chemistry, so eat them raw.
Day one: within 24 hours the benzene and acrolein byproducts in your urine are rising above baseline — real, and invisible to you 3.
Weeks four to twelve: if you started with high blood pressure, insulin resistance, or a raised inflammation marker, those readings drift the right way. You notice only when someone shows you the graph.
Years: here it turns from measured to inferred. Cruciferous vegetables carry the steepest cardiovascular-mortality benefit of any vegetable group in the largest pooled cohorts 7, and sprouts are its concentrated form. No trial will ever prove this for sprouts alone; the bet is that two minutes a day is worth it.
The fine print — when to skip it, and what people get wrong
The real risk is foodborne illness, not the compound. Raw sprouts grow in warm, damp conditions that also favor Salmonella, E. coli, and Listeria; contaminated seed has caused repeated outbreaks 8. Skip raw sprouts in pregnancy, in young children, over about 65, or if immunocompromised — use cooked broccoli with mustard powder instead. Bloating or loose stools at higher intake fade if you start at a quarter-cup for a week.
"A pill is the same as the food." Only if it lists active myrosinase or pre-formed sulforaphane. Cheap supplements hold glucoraphanin alone and lean on gut bacteria to convert it, dropping the dose to about a quarter with wide person-to-person swings 9.
"Broccoli hurts your thyroid." Not at these doses. A 12-week trial found no shift in thyroid hormones or antibodies 10. And more is not better: the pathway saturates, so doubling the dose does not double the effect.

Broccoli Sprouts and Sulforaphane
Home-sprouted: ~$15 for 0.5 lb organic broccoli sprouting seed (~3-month supply), $0.20–0.50/serving. Commercial fresh sprouts $3–6/100 g. Reputable supplements with active myrosinase or pre-formed sulforaphane: $20–40/month. Trivial cost in the home-sprout case.
Home sprouting: overnight soak plus 2x daily rinses for 3–5 days, then refrigerate; ~2–3 min/day. Daily fresh consumption (chewing thoroughly so myrosinase activates) is the constraint, not the prep. Supplement route is one capsule daily but requires picking a formulation with active myrosinase or pre-formed sulforaphane to actually deliver the dose.
Pooled meta-analysis of 10 RCTs shows ~10.9/6.95 mmHg BP reduction in cardiometabolic populations over 4–12 weeks (Li 2022); hs-CRP and IL-6 fall on the same timescale (Mirmiran 2012); fasting insulin and HOMA-IR improve in T2D at 10 g/day sprout powder (Bahadoran 2012). Clear functional improvement in clinical subgroups; smaller, less consistent in healthy normotensives.
Mechanism is the most potent dietary Nrf2 inducer characterised, with replicated acute carcinogen detoxification in humans (Egner 2014: +61% benzene, +23% acrolein urinary clearance) and contribution to the Qidong aflatoxin-detox programme that preceded a 75% regional liver-cancer incidence decline. Cruciferous-vegetable subgroup carries the steepest CV-mortality dose-response in the largest pooled cohort analyses (Aune 2017). Clinical-endpoint cancer RCT does not exist.
Mechanism (Keap1/Nrf2/ARE induction) is settled (Kensler 2013). Acute carcinogen detoxification is replicated in multiple Qidong RCTs (Egner 2011, Egner 2014). BP effect rests on a meta-analysis of small trials, predominantly in cardiometabolic populations (Li 2022). Notable negative: COPD RCT found no Nrf2 induction in lung tissue (Wise 2016). Prostate trial missed primary endpoint (Cipolla 2015). Settled mechanism + small positive trials + one negative trial = 3.
