A cleanser can't tell your skin's oil from the dirt. The chemistry that lifts soil away strips the lipids holding your outer skin layer together. Two label details set how much you strip.
The surfactant. SLS is the harsh one, raising water loss through skin two to three times after one exposure 1. SLES is gentler but still stripping 2. The mild end is the betaine and sugar glucosides "sulfate-free" formulas blend 3.
The pH. Skin sits slightly acidic, pH 4.5 to 5.5, and its enzymes and flora depend on that 4. Bar soap runs pH 9 to 10, and plenty of supermarket liquids measure just as high 5.
The barrier damage is real and reversible. A typical sulfate cleanser raises water loss and thins the outer layer; switch to a milder one and it normalizes within days to weeks 6.
Fragrance is the real hazard in the bottle. Among people patch-tested for unexplained rashes, fragrance mixes come back positive in about one in nine to one in twenty, near the top of all contact allergens 7. A US label hides dozens of them behind one word, "fragrance" 8.
Fix the product and the pattern. Read the ingredient list once, then use less soap on less of you.
Week one, little changes except the shower feels less squeaky-clean — that squeak was stripped lipids. Weeks two to four, any low-grade dryness eases and the shins stop itching at night. Months in, you stop noticing your skin, which is the point. The years-long payoff was never put through a trial; it rests on the mechanism and on what dermatologists tell reactive patients.
Who feels it fastest: anyone with eczema, rosacea, or reactive skin, for whom a fragranced sulfate wash is part of the problem 13; people over 65, half of whom have dry skin as their lipid film fades 14; and anyone in a dry-winter climate.
The fine print — when to skip it, and what people get wrong
"Natural" isn't gentle: bar soap is about as natural as cleansers get and among the harshest, and "natural" washes lean on essential oils, the top fragrance allergens 5. Ordinary wash doesn't wreck your microbiome; flora shift for hours, then snap back 11. The "antibacterial" washes that did shift it were pulled years ago 12.
If switching didn't help: you changed the brand but kept the twenty-minute whole-body hot shower, and volume beats product choice 10; or the culprit was your laundry detergent, moisturizer, or perfume, not the wash at all.

Body Wash
Mainstream syndet body wash runs $20–60/year; dermatology-recommended brands $40–120/year. Modestly above bar soap, trivial in any household budget.
One-time decision: read the ingredient list, swap the product, set up a reorder. Zero ongoing effort in use — the cleanser was already being used; only its identity changes.
Cleanser-induced barrier disruption is one of the more replicable findings in cosmetic dermatology — controlled TEWL/corneometry studies, mechanism (lipid extraction, protein denaturation, pH disruption) is well-characterized, and AAD/NEA practice guidance is aligned (Ananthapadmanabhan et al. 2004; Korting et al. 1990; Schmid-Wendtner & Korting 2006). Fragrance contact allergy prevalence is established by large patch-test datasets (DeKoven et al. 2018).
Switching from a harsh, fragranced, high-SLS body wash to a mild low-SLS or sulfate-free pH-balanced formulation visibly reduces redness, dry patches, and reactive skin within 2–4 weeks. Controlled exposure studies show measurable TEWL elevation and erythema with anionic surfactants that normalizes within days of removing the irritant (Ananthapadmanabhan et al. 2004; Löffler & Happle 2003).
Felt-experience improvement within weeks for sensitive-skin and atopic readers — less itch, less tightness, fewer reactive episodes. For unreactive skin the change is subclinical. Aggregated across the population, a real-but-small daily quality-of-life lift (NEA 2023; AAD 2024).
Daily exposure compounds: decades of mild surfactant insult contribute to chronic xerosis and low-grade dermatitis, especially over 50 when stratum corneum lipid synthesis declines. The contribution is real but small versus dominant cumulative levers like UV exposure (Hillebrand et al. 2008).
