But the one folic-acid trial moved speech-range hearing by only 0.7 dB over three years, too small to hear, and it ran in a country that doesn't fortify its food, so it barely transfers to anyone eating fortified cereal 2.
For tinnitus, the split is sharp. B12 injections eased chronic ringing only in the people who were deficient to start; everyone else, and placebo, got nothing 3. B12 helps the people who need B12, and no one else.
This is for a few groups, not everyone. B Vitamins and Hearing

Plausible mechanism (homocysteine-driven cochlear vascular damage plus direct oxidative injury — Martinez-Vega et al. 2015, 2017), consistent cross-sectional associations across multiple populations (Houston 1999, Lasisi 2010, Gopinath 2010), and one positive RCT (Durga 2007, 0.7 dB slowing of speech-frequency decline over 3 years). But effect sizes are small, null replications exist (Berner 2000), and the only RCT was in a non-fortified population. Worth acting on in deficient or at-risk individuals; monitor for confirmatory trials.
In B12-deficient patients, repletion resolves fatigue, paresthesias, and (when present) tinnitus severity over weeks (Singh et al. 2016 pilot, n=40; Shemesh et al. 1993). No felt change in replete adults. Real but small at the population level — concentrated in the at-risk subgroup the entry targets.
Two converging pathways: untreated B12 deficiency causes irreversible neuropathy, cognitive decline, and dementia on a years-timescale; and hearing loss itself is one of the largest modifiable dementia risk factors. Correcting deficiency in risk groups (60+, vegans, metformin/PPI users) prevents both. Small additive effect on mortality.
B12 deficiency reliably causes fatigue via macrocytic anemia and neurological effects; repletion restores energy in deficient patients (NIH ODS B12 2024). Effect is real and noticeable in the deficient subgroup, trivial in replete adults.
